I’ve reviewed thousands of labs across every stage of health: acute trauma, chronic fatigue, unexplained inflammation, elite performance, and everything in between. And here’s the uncomfortable truth: most men walking into a clinic look fine on paper—until you look at the right paper.

We live in a world that encourages guesswork. Vague advice like “eat better, exercise more” gets thrown around as if it means something. Meanwhile, symptoms like fatigue, brain fog, low libido, poor recovery, or creeping weight gain get brushed off as aging—or worse, laziness.

Beyond Testosterone: Why TRT Blood Tests Should Be Comprehensive

At Atlas Method, we start with comprehensive blood testing because I’ve seen what happens when we don’t. I’ve treated men who were told everything looked “normal,” yet couldn’t make it through the day without crashing. Their numbers were within range—just not the ranges that matter.

Here’s what we test, and why. This isn’t an exhaustive list, but these are the patterns I’ve seen over and over again—the ones that separate high performance from hidden dysfunction.

We don’t stop at HDL and LDL. We measure Apolipoprotein B (ApoB)—a far better predictor of cardiovascular risk. According to JAMA Cardiology (Sniderman et al., 2022), ApoB is a more reliable marker than traditional cholesterol metrics. Elevated ApoB isn’t just a number; it’s often a red flag for early atherosclerosis—even in “fit” men.

Total Testosterone, Free Testosterone, and SHBG

Testosterone matters. But I’ve seen too many men fall through the cracks because their doctor only ran Total Testosterone. At Atlas, we also measure Free Testosterone—what’s actually usable by your body—and SHBG, the protein that binds it.

When Free T is low and SHBG is high, you’re going to feel it—no matter what your total number says. The Journal of Clinical Endocrinology & Metabolism backs this up (Morgentaler et al., 2015): Free T is a more accurate indicator of symptoms than Total T alone.

Estradiol and DHEA-S

Estrogen isn’t just a “women’s hormone.” Men need balanced levels of Estradiol for joint function, mood stability, and sexual health. I’ve seen elevated or suppressed estradiol wreck performance in otherwise healthy men.

DHEA-S, a precursor hormone made by the adrenals, gives us insight into stress resilience. Chronically low levels? That’s a sign your system’s been running on fumes for too long.

Thyroid Panel: Free T3 and Thyroid Antibodies

I’ve lost count of how many men were told their thyroid was “fine” based on a single TSH reading. It’s not enough.

We run Free T3 and screen for thyroid antibodies to catch early dysfunction—especially Hashimoto’s and subclinical hypothyroidism. A 2014 study in JCEM (Biondi & Wartofsky) confirmed Free T3 is a more reliable marker for clinical performance than TSH.

Hemoglobin A1c

If your blood sugar is spiking and crashing, you won’t feel stable—mentally or physically. Hgb A1c gives us a three-month average of how your body is handling glucose. If it’s trending up, we act early. It’s not just about diabetes prevention—it’s about optimizing focus, metabolism, and mood.

CMP (Comprehensive Metabolic Panel)

This test checks liver enzymes, kidney function, glucose, and electrolytes. I consider it the foundation. I’ve seen seemingly “clean” labs hide the early signs of liver stress, dehydration, or insulin resistance. The CMP helps us catch it before it becomes symptomatic.

CBC (Complete Blood Count)

This test gives a snapshot of your blood’s health—red and white cells, platelets, inflammation markers. I look at this when men report fatigue, slow recovery, or frequent illness. Often, there’s more happening than meets the eye.

Vitamin D3 and B12

Two of the most overlooked metrics in men’s health. Vitamin D3 isn’t just for bones—it’s essential for testosterone production and immune regulation. B12 affects energy, cognitive clarity, and neurological function. I’ve seen both deficient in men who eat well and spend time outside. You’d be surprised how common this is—even in Florida.

PSA (Prostate-Specific Antigen)

We run PSA early and often. Prostate issues don’t start at 60. I’ve seen enough unexpected elevations in healthy men in their 30s and 40s to know that monitoring this number matters.

What TRT Blood Tests Teach Us

I don’t treat numbers. I treat people. But over the years, I’ve found that patterns in the blood often explain what someone’s been trying to put into words for months—or years.

One patient came in with textbook labs from another clinic—except no one had checked his SHBG. It was sky-high, binding up nearly all of his testosterone. Another had chronic fatigue and mental fog. No one had tested his Free T3. We adjusted his thyroid levels and within weeks, he felt like himself again.
